Addiction and Mental Health Treatment in Old Hickory, Tennessee: A Local Guide
Old Hickory sits on Hadley Bend, a loop of the Cumberland River in far northeastern Davidson County, about fourteen miles from downtown Nashville and a few minutes from the Old Hickory Lock and Dam. It is part of Metro Nashville, but it does not behave like the rest of the city. The ZIP code shared with the former town of Lakewood, 37138, covers roughly 23,600 residents with a median household income near $85,000 and a median age of about 43, older and steadier than Davidson County overall. That combination shapes how people here reach treatment: most households have a car and an insurance card, and almost nobody has a clinic within walking distance.
We at TreatmentTennessee.com built this section to sit underneath the facility listings above and answer the questions those listings cannot: what the county data actually shows, how far you will realistically drive, what care tends to cost here, and when Old Hickory is the wrong place to get treatment. The listings are a starting point. What follows is the local context that helps you use them.
The Village
The original DuPont company town, platted in 1918 around the powder plant and rebuilt for rayon workers in 1925. Bungalows along Hadley Avenue, Jones Street and Riverside Drive form a National Register historic district. Dense, walkable, and served by the Old Hickory library and community center.
Lakewood and Rayon City
South of the Village along Old Hickory Boulevard. Lakewood was its own incorporated city from 1959 until residents voted in 2011 to surrender the charter and fold into Metro. Older housing stock, lower rents, and the commercial strip at 22nd Street.
Riverside and the Dam
Old Hickory Boulevard, State Route 45, crosses the dam into Sumner County. That crossing puts Hendersonville closer than most of Nashville, and it means many residents split their medical care across two counties and two hospital systems.
What the Davidson County data shows
Old Hickory is too small for the Metro Public Health Department to publish neighborhood-level overdose counts, so county figures are the closest reliable measure. Those figures have moved sharply in one direction over the past four years.
Suspected fatal drug overdoses in Davidson County peaked at 725 in 2021 and stayed above 700 through 2023. The county recorded 513 suspected overdose deaths among residents in 2024, an age-adjusted rate of 71.4 per 100,000, and the Metro Public Health Department reported more than 400 suspected deaths in 2025. Through the first quarter of 2026, 80 people had died, which the department described as a 35 percent decline against the same period a year earlier.

Counts from Metro Public Health Department surveillance reports. All figures are provisional and revised in later reports.
The 2026 bar covers three months only and is not comparable to the full-year bars above it.
Three numbers worth knowing before you choose a program
Fentanyl is in roughly seven of ten fatal overdoses here. Metro Public Health Department toxicology data has put fentanyl detection near 70 percent of suspected overdose deaths for several consecutive reporting years. A program that cannot start buprenorphine or methadone, or refer you the same week to someone who can, is not matched to the local drug supply.
Most deaths involve more than one substance. Cocaine and methamphetamine appear alongside fentanyl in a large share of county cases, and xylazine has shown up in the emerging-substance data. Ask any facility how it handles stimulant use, since medications for opioid use disorder do not treat it.
Between 8.1 and 10.2 percent of Tennessee adults were uninsured in 2025. That estimate comes from the University of Tennessee Boyd Center survey conducted for TennCare. Tennessee has not expanded Medicaid, so a working adult in Old Hickory who earns too much for TennCare and buys no marketplace plan usually pays out of pocket or waits for a state-funded slot.
Where the care actually is
There is no hospital in Old Hickory and no residential treatment campus inside the 37138 boundary. What Old Hickory has is position. It sits inside a ring of four treatment markets, each roughly ten minutes away in a different direction, and each governed by different insurance networks and different county systems.
South: Hermitage and Donelson
Follow Old Hickory Boulevard or Robinson Road south to Lebanon Pike and Interstate 40 at exit 221. TriStar Summit Medical Center on Frist Boulevard is the nearest full-service hospital and emergency department. The Donelson and Hermitage corridor holds most of the outpatient behavioral health offices, opioid treatment programs and intensive outpatient schedules that Old Hickory residents actually use.
West: Madison, Rivergate and Inglewood
Old Hickory Boulevard west toward Gallatin Pike reaches Madison in about fifteen minutes. This is where Metro’s transit density begins, where several community mental health providers operate, and where the closest inpatient behavioral health beds and state-funded outpatient slots tend to sit. It is also the direction with the most sliding-scale options.
East: Mt. Juliet and Wilson County
Part of the 37138 postal area already falls in Wilson County. Crossing into Mt. Juliet or Lebanon changes your drug court, your public defender’s office and sometimes your provider network, but it can shorten a wait list. Vanderbilt Wilson County Hospital in Lebanon is the anchor facility on that side.
North: Hendersonville and Sumner County
Cross the dam and you are in Sumner County within about ten minutes. Hendersonville and Gallatin have their own outpatient programs and hospital. For someone who wants distance from a small, tight community where the DuPont families have known each other for three generations, this is often the shortest route to privacy.

Getting there without a car is the hard part
This is the single biggest practical obstacle to outpatient treatment in Old Hickory, and it is the thing generic rehab pages never mention. Old Hickory Village has no direct fixed-route WeGo bus running through it. Historic Old Hickory Village, the neighborhood association, notes that the nearest park-and-ride locations are in Madison and Hermitage, and that parts of Old Hickory qualify for WeGo Link, which subsidizes a rideshare trip to a bus stop. Metro’s Choose How You Move program is expanding service, sidewalks and reduced-fare eligibility, but the buildout is gradual.
The practical consequences are specific. An intensive outpatient program that meets three evenings a week for twelve weeks is roughly thirty-six round trips. Losing a driver’s license after a DUI, or losing a car, can end a treatment episode in Old Hickory in a way it would not in Inglewood or Midtown. Before you enroll anywhere, work out the transport for the full length of the program, not the first week.
Three questions to settle before the first appointment: Does the program offer a telehealth option for some sessions, and does your insurance cover it at the same rate? Does it provide transportation assistance, which some state-funded providers do? Can you park on site without a fee, which matters more at midtown hospital campuses than at Hermitage or Madison offices?
Paying for treatment from a 37138 address
Old Hickory’s income profile changes the funding conversation. With a median household income around $85,000 and a poverty rate near 8 percent, a large share of households here hold employer coverage through DuPont-successor manufacturers, Metro Schools, healthcare employers or the trades. That is an advantage for access and a disadvantage for cost, because commercially insured patients face deductibles and coinsurance that TennCare members do not.
Our TreatmentTennessee.com directory keeps separate pages for the plans most often presented in Middle Tennessee, and verifying network status before admission is the step that prevents most surprise bills.

What treatment tends to cost in this part of Middle Tennessee
Published self-pay prices vary widely and change often, so treat these as bands to check rather than quotes. In the Nashville market, standard outpatient counseling commonly runs from about $100 to $200 per session. Intensive outpatient programs are often quoted as a weekly or full-program rate rather than per session. Partial hospitalization sits above that, and medically supervised withdrawal management and residential care are the most expensive levels, usually quoted per day.
Medications for opioid use disorder are a separate line. Office-based buprenorphine typically involves a prescriber visit plus the pharmacy cost of the medication. Methadone requires daily dosing at a licensed opioid treatment program, which changes the transport math considerably for an Old Hickory resident.
Ask every facility for the same four figures in writing: the all-in program price, what the insurance-negotiated rate would be, your estimated out-of-pocket after deductible, and what happens financially if you leave early or need a longer stay. Federal parity rules require most plans to cover behavioral health on terms comparable to medical care, and the No Surprises Act gives self-pay patients the right to a good faith estimate.
If you have no insurance
Tennessee has not expanded Medicaid, so many working adults in Old Hickory fall between TennCare eligibility and affordable marketplace coverage. The state runs the Behavioral Health Safety Net through the Department of Mental Health and Substance Abuse Services, which funds outpatient mental health services for eligible uninsured adults and children through a network of community providers covering all 95 counties. Substance use services for uninsured Tennesseans are funded through separate state contracts, and eligibility is assessed by the provider.
The Metro Public Health Department also operates the Community Overdose Response Team, a free and confidential service that helps people at risk of overdose find treatment at the appropriate level of care regardless of insurance status. The Lentz Public Health Center offers no-cost substance use assessments and brief mental health screenings by appointment.
How to verify a facility before you call
Licensure and accreditation are two different things, and both are worth checking. Residential and non-residential substance use facilities in Tennessee are licensed by the Department of Mental Health and Substance Abuse Services. Accreditation is voluntary and comes from bodies such as The Joint Commission or CARF International, each of which publishes a public search tool.
Confirm three things by phone before you commit: that the facility is currently licensed for the level of care you need, that the clinician who will treat you is licensed in Tennessee, and that the specific program, not just the parent organization, is in network with your plan. Facilities change ownership and network status frequently, and the directory listing on any website, including ours, reflects the last time the record was updated.
The honest trade-offs of treatment in Old Hickory
What works in your favor
You have four separate treatment markets within a fifteen-minute drive and can shop across county lines when one has a wait list.
Metro Nashville’s overdose surveillance, naloxone distribution and response programs cover you as a Davidson County resident even though you live on the county edge.
Housing is quieter and more stable here than in central Nashville, which supports outpatient recovery when the home environment is safe.
The Village has genuine community infrastructure: a library, a community center, churches, a chamber of commerce and long-standing neighborhood networks that peer support can plug into.
What works against you
Transit is thin. Without reliable transport, an evening intensive outpatient schedule is difficult to sustain for twelve weeks.
Privacy is limited in a community this small and this old. People recognize cars in parking lots.
Specialist care concentrates in midtown. Adolescent programs, eating disorder care, dual-diagnosis inpatient beds and perinatal substance use programs generally mean a drive across the county.
The county line running through the postal area creates confusion about which court, which crisis team and which provider network applies to you.
Who Old Hickory suits, and who should look elsewhere
Staying local usually makes sense if:
You have a car, a stable home and a job you want to keep, and outpatient or intensive outpatient care is the recommended level.
You are stepping down from a hospital stay or withdrawal management and need continuity close to home.
You are managing a medication for opioid use disorder and need a prescriber you can reach quickly.
Family involvement is part of your plan and relatives live in the 37138 or Hermitage area.
Looking beyond Old Hickory usually makes sense if:
You need medically supervised withdrawal management or residential care, neither of which exists inside the neighborhood.
Your home environment includes active substance use, and remaining in it undermines any outpatient plan.
You need a specialty track: adolescent care, co-occurring serious mental illness, pregnancy, or a program in a language other than English.
Anonymity matters more to you than proximity, in which case Hendersonville, Rivergate, Mt. Juliet or the Nashville core all put useful distance between treatment and your street.

If the situation is urgent
Call 911 for a suspected overdose or any medical emergency. The nearest emergency department to Old Hickory Village is at TriStar Summit Medical Center in Hermitage.
Call or text 988 to reach the Suicide and Crisis Lifeline, which routes to Tennessee crisis services and can dispatch mobile crisis teams.
Call the Tennessee REDLINE at 1-800-889-9789 for free, confidential referrals to alcohol, drug and problem gambling services anywhere in the state, with or without insurance.
Call the Community Overdose Response Team at 615-687-1701 to be matched with a level of care in Davidson County at no cost.
Before you choose
Contact at least three facilities. Ask each one the same questions about level of care, licensure, network status, medications offered, wait time and total cost, and write the answers down so you can compare them side by side. A program that answers those questions clearly on the phone tends to be clearer about everything else too.
Independent verification matters more than any directory, including this one. Check licensure with the Tennessee Department of Mental Health and Substance Abuse Services, check accreditation with The Joint Commission or CARF, and cross-reference the federal treatment locator run by SAMHSA. Then call your insurer directly rather than relying on the facility’s word about coverage.
The listings above this section give you the Old Hickory starting point. TreatmentTennessee.com maintains this directory as a research tool, not a referral service, and we do not rank or recommend individual providers. The decision belongs to you and the clinician who assesses you.
